为什么内?
如果填with a sword in his hand对不对??

可以，不过如果要用with的话，可以不用逗号的。
这里用sword in hand 比较好，显得更符合英语简洁的习惯。

或
The captain walked in front of the line with a sword in his hand.
或
With a sword in his hand，the captain walked in front of the line .

以上三种都可取

,的作用.后面是独立句子,补充说明主语的状态.
