当然第一句对了.that 本来就作为指示代词了后面就不需要加冠词了.
Could you please give me that book?请给我那本书,好吗?
Could you please give me that book?请给我一本书,好吗?

That book已经指定了是"那一本书"而不是其他的

A book表示是随便的一本
Could you please give me a book? 就是你能不能随便给我一本书, 就不可能再用that去指定是哪一本了.

or
could you please give me that book
