Which one should we Chinese choose

ls...this top 5% does not indicate whether the counselor recommend the applicant or not! This appears in the section where a counselor is supposed to report the applicant's ranking in the whole graduate class, if you rank among the top 5%, then choose the top 5%; however if you fall out of this category and land into top 10%, then go ahead and choose 10%. Never report fake scores!! It's not a matter of CHOOSE which one, it's a matter of REPORTing!! Be honest!

And as to what ls has said, "no basis" means the counselor or teacher does not have a clue on how you behave on this aspect. Anyway, no matter what you choose, you should NEVER choose "no basis". Just think about this: why would you choose someone who do not even have a "basis" to know how you behave to write a recommendation?

Of course you should choose the top 5%...
No basis means the professor or counselor doesn't intend to recommend this candidate at all...
