What happened?
Technician was removing cap from H2O2 tote bottom valve and it ruptured.
Why?
The original cap for the peroxide tote was misplaced and a cap from another tote was used to replace it. The tote bottom valve leaked. H2O2 was decomposing in the small volume between the valve and the cap due to contact with contaminants. Oxygen gas generation was resulting in pressure buildup in the void. When the technician started to remove the cap is finally ruptured.
