3.日期：4月6日；
4.其他：表示将尽快返校，补上所缺课程；
5.至少五句话。

我尽量写的幼稚一点：
Hi teacher, I am Li Na. Because my grandma is fever and my parents are both in business trip, may I have 3 days leave from Apr 6th, to take care of my grandma. I will try to come back to school as soon as possible, and learn the classes I missed.
