经甲乙双方共同协商,就乙方承接甲方健身房淋浴间塑钢泥防水边制作工程事宜达成如下协议:

付款方式:
合同生效后甲方需先支付30%预付款计壹万陆仟捌佰零伍元贰角伍分整
工程全部完工后,甲方需在两天内组织验收,验收合格后十天内向乙方支付工程款的65%计叁万陆仟肆佰壹拾壹元叁角柒分整，余款5%计贰仟捌佰元捌角柒分整作为质保金在质保期满十二个月后七日内付清。
备注:
1 乙方施工人员的午餐、晚餐由甲方负责:
2 甲方以汇款或现金方式支付乙方工程款时,凭发票为准:
3 甲方需按协议付款期及时付款,如因甲方原因造成工程不能如期完工,则甲方应提前支付工程款给乙方:
4 工程保用期为伍年,保用期内如因乙方原因需维修的,乙方将负责免费维修:如因甲方原因造成损坏的,乙方给予维修,只收取适当的材料费:
5 施工过程中因故造成技术人员伤残.医疗费用将由乙方承担;
6 如乙方人员施工过程中对甲方物品造成损坏,由乙方负责赔偿.
7 乙方施工所用材料必须采用SH-801型塑钢泥，施工前需经甲方确认。

以上条款未尽事宜,须经甲乙双方共同协商,达成一致后作为补充条款,补充条款与此合同具有同等法律效力.
此合同一式两份,甲乙双方各执一份
