语态都搞错了，memory怎么能主动remind呢？应该是人remind memory，而不是memory reminds人；而wishes go into your heart就不需要用被动了。如果你想做小改，那么可以改为：
let the sweet memory be reminded in our minds; let my best wishes go into your heart.

个人觉得这样好一点
Let the sweet memory keep in our minds, let my best wishes be in your heart.

let our sweet memory remains in our mind ,let my best wishes stay in your heart这样会不会比较好啊
