She is a first grade student.
first grade 一起作定语，短语的中心语是student 前面有冠词a，这时不加。
She is a student in the first grade.
in the first grade介词短语做后置定语，要加。
She is in the first grade。介词短语作表语，也要加。
不用加the
强调第一的时候，一定要加the；不是第一或者不强调第一的时候，不加the。这里的a first grade student是一年级学生的意思，没有第一的意思。
first前面有 a 的时候当然不加。因为有形容FIRST的了
