（英文）工作经验证明信格式是什么？

目前需要给一名前采购主管开一封英文的工作证明信，需要写明其姓名，性别，出生年月日，职位，薪水，工作时间和表现。该主管在我公司工作2年，第一年是采购助理，后来提升为主管，薪水也有涨。
请问如果开证明信，标准格式是如何的？急，请知情者帮忙答复。
