急求英文版的六级证书格式

如题，需要把手上的CET-6翻译成英文版。据说是有标准格式的，可是我找了很久也没找到。

没听说有格式，不过我翻译过，希望能对您有用。

COLLEGE ENGLISH TEST (BAND 6)

Let it be known that Xing Wenxiang, a student of grade 91 in the Department of National Economic Management of Liaoning University, took part in State College English Test Band 6 in Jun. 1994.
