我认为应该是A．因为WHO后面的动词不是要用单数吗

who 只是定语从句的一个连词，它引出了are dying of the disease，从而修饰people， people是复数形式的，所以选b，没有错。整句话可以转化成how many people who are dying of the disease does the doctor know?但是显得比较累赘，所以谓语前置。
