如果是倒装的话,not only一定在句首
但是并不是所有在句首的not only都倒装的
eg. Not only he but also I like singing.
在句首时,
当not only后面只有一个名词短语的时候,不用倒;
当not only后面是一句句子的时候,要倒装

not only is he handsome, but also he is very polite

Not only can he play football,he can also play basketball

只有not only 置于句首的时候才需要倒装.
如:
Not only he is a doctor, but also can he cook well.
他不但是个医生,而且厨艺也不错.
He can not only cure others' diseases, but also cook well.
他不但能治愈他人的疾病,而且做一手好菜.

not only只是在它后面接一个句子的时候才倒装，而且是部分倒装，也就是谓语动词的一部分移到主语的前面。我是英语老师，前面说的都有一定的错误。
