留学工资收入证明表格/格式？

我要去新西兰留学了，现在正在办理签证中...（我没有找中介，是自己在办）
但是需要开一份工资证明，家长的单位说要有固定的格式，请问是什么样的呢？或者有什么表格吗？

我司给我的工资证明如下：
