第一第二句错误，prove表示“被证明什么（跟上形容词）”，一般都是主动，只有解释为“。。。得到了证实”，才用被动，比如His theory has been proved.第三第四句对，to be在很多词组里都可以省略。
都对
应当都对吧。只是习惯上第1、4种更加常用吧。
