目的：提高门诊处方质量，减少门诊不规范处方的发生率，促进合理用药，保障医疗安全。 方法：分析我院2007年5月1日至2008年2月29日十个月门诊处方总量1118577张，发现不规范处方723张，占总处方0.065％。 结果 常见不规范处方有：剂量不规范，占不规范处方总数的47.44%；电脑录入不规范，占不规范处方总数的24.62%；用药次数不规范，占不规范处方总数的16.46%；用药部位不规范，占不规范处方总数的8.3%；选药不规范，占不规范处方总数的1.38%；其它不规范处方占不规范处方总数的1.80%。 结论： 我院门诊不规范处方的发生率较低，但也直接影响病人的用药, 个别不规范处方已造成不良医疗后果，引起医疗纠纷。药剂师应帮助临床医师查找门诊不规范处方发生的原因，制订相应的对策；提醒和督导临床医师安全、合理、有效、正确的用药，以杜绝药物不良反应和药源性疾病的发生。
