摘 要
目的：探讨肝硬化患者上消化道出血的时间规律，从而制定相应的护理对策，为预防和治疗提供依据。 方法：本研究根据住院期间的医疗护理病历以调查表的方法对190例肝硬化患者发生418例次上消化道出血时间进行了回顾性调查。发现肝硬化患者发生上消化道出血的时间规律，以便在以后的临床工作中，为临床护理提供参考，加强对出血高危人群的观察和护理。 结果：24小时中夜间5＿6时为出血第一高峰，晚间16＿17时为出血第二高峰；418例次出血中，185例次出血发生在夜间（0＿7时），74例次出血发生在日间（8＿15时），159例次出血发生在晚间（16＿23时），3个时间段出血例次比较，夜间最高，晚间次之，日间最低，差异有显著意义（P＜0.01）；190例肝硬化上消化道出血患者中，因出血死亡34例，死亡率17.9%，死亡数在各时间段的分布是夜间18例，晚间14例，日间2例，3个时间段死亡发生率比较，夜间最高、晚间次之、日间最低，差异有显著性意义（P＜0.05）。 结论：研究结果发现肝硬化患者发生上消化道出血的时间及死亡时间有规律性变化。 据此提出相应的护理对策：加强出血高发时间段的病情观察；加强出血高危人群的防护措施；减少出血发生率，提高抢救成功率和降低出血死亡率。
