in no case 在任何场合都不……等于“not ……in any case”

You can smoke in no case .你在任何场合都不能吸烟。

by no means 决不，一点也不。等于on no account

I am by no means interested in the work．我对这件工作一点也不感兴趣。

He will by no means lend you the money．他决不会把钱借给你的。

in no case大概就是在任何场合（情况）下都不能
by no means意思等于on no account，但是用法不一样。
by no means: be + by no means + v
on no account: on no account + can sb + v
纯属个人意见，仅供参考。如果不是用于应试，就不用太计较这些。语言是工具，不是监牢。

至于第一个问题……我怕说了完全误导……

因为should we do 是一个差入语，可以拿掉的，我想高中老师都讲过的，比如 what do you think is best ?中的 do you think 就是插入语，可以去掉的，再说后面的 ，因为是讲的明天应该是没有发生的，是虚礼语气所以选c拉，呵呵，还可以吧
