注意事项
一、 本证加盖认定机构公章后生效。
二、 本证为持证人具有教师资格的法定凭证。持证人应妥善保存，不得涂改、损毁。如有遗失，应立即向发证机关报告。
三、 持证人丧失教师资格或者被撤销教师资格，其教师资格证书由县级以上人民政府教育行政部门收回。
