[摘要]目的：比较齐拉西酮与氟哌啶醇治疗精神分裂症急性期的疗效与副作用。方法：选用齐拉西酮注射剂与氟哌啶醇注射剂，对75例患者随机分成齐拉西酮组（n=38）和与氟哌啶醇组（n=37），进行为期5天的治疗。分别在治疗前及治疗后采用阳性与阴性症状量表（PANSS）、临床总体印度量表（CGI）及不良反应量表（TESS）评定疗效及药物不良反应。治疗前进行基线心电图测定，与治疗后QTc间期比较，超过基线60ms者为QTc间期延长①,对治疗过程中体格检查发现心律异常者及时监测心电图。结果：齐拉西酮组和氟哌啶醇组治疗前后PANSS总分和阳性症状量表因子分减分率相当；CGI评分达到好转与以上者分别为25例（73．5％）、26例（78.8%）;TESS评定副反应发生分别为10例（29.4%）12例（36.36%）。经T检验均无统计学差异（P>0.05）。齐拉西醇组主要副作用为失眠，氟哌啶醇组主要副作用为锥体外系反应，但齐拉西酮组有3例（7.89%）出现QTc间期延长，2例（5.26%）出现心律失常;氟哌啶醇对心电图影响多为室性心率不齐，未发现明显QTc间期延长者。结论：齐拉西酮注射剂与氟哌啶醇注射剂对治疗精神分裂症急性期的疗效相当，但齐拉西酮组对心脏QTc间期的影响及是否引起心率失常需引起高度重视。
