摘 要

目的：研究自身免疫溶血性贫血伴同种抗体引起输血反应。方法：ABO血型、Rh血型鉴定、Coomb’s试验、吸收放散试验、抗体筛选及鉴定、效价测定、自身抗体检查及效价测定。结果：患者血型O、CcDEE，供血者血型O、CCDee，直接抗人球蛋白试验阳性，多特异性抗人球蛋白3+、抗IgG2+、抗C33+，通过吸收放散试验，自身抗体检查为温性自身抗体。吸收后血清同种抗体检查，证实血清中含有抗-e抗体。结论：RH-Hr血型系在免疫血液中的重要性仅次于ABO血型系统，但临床上多见于抗D，其次为抗E、抗C、抗c和一些复合性抗体，而抗e为最罕见。本例为AIHA患者，反复输血，在长期输注过程中，因而受到e抗原的免疫而产生了抗e抗体。本次住院输注洗涤红细胞供者血型为O、CCDee。患者输注后，产生免疫应答，造成了严重的输血反应。体内免疫能力异常，除了产生自身抗体外，对输入的其它不相合抗原极易产生抗体，自身抗体给血型鉴定及交叉配血都带来一定困难。这类病例只有采取自身吸收的方法除去自身抗体，进一步鉴定同种抗体特异性，方能选择相容的供者。本例患者经查明为温型AIHA伴同种抗体抗e。进行献血员的筛选时，选用不含e抗原的洗涤红细胞输注4u，均未发生不良反应，Hb逐渐上升至62g/L，病情基本稳定。
