仁济医院静脉药物配置中心常见的不合理用药分析
[摘要] 目的：分析静脉药物配置中心（PIVAS）在药师审方过程中常见的不合理用药。
方法：对仁济医院2008年第一到第三季度PIVAS审核的527820张医嘱中发现的971例不合理用药纪录进行统计分析。
结果：属于不合理用药医嘱共有971例占全部医嘱的0.184%。静脉药物输液存在药物配伍不当占427例（43.98%），其中全营养混合液（TPN）中存在的问题10例占总TPN总量的0.078%，溶媒选择不当占30例（30.99%），药物浓度稀释不当108例（11.12%），药物超剂量占135例（13.90%）。
结论：药师通过PIVAS这个平台，经过审方，发现临床习惯性用药中的不合理用药现象，并通过干预，及时纠正，减少了药物的不良反应，保证了临床用药的安全、有效。
