急性双表型白血病（BAL）是一种少见类型的白血病，其具有独特的临床、生物学和预后特征。BAL的诊断和治疗均不同于单一类型的白血病。因此，我们回顾性分析了6例BAL患者，这些患者均是根据国际白血病欧洲协作组（EGIL）1995年标准而诊断的急性双表型白血病。在本次研究中，我们对这6例BAL骨髓标本分别进行光镜细胞形态学及相关细胞化学染色观察，以确定其FAB类型；应用流式细胞仪做免疫分型，即使用一系列相关单克隆抗体检测细胞膜和细胞质抗原表达情况；同时采用G显带技术进行核型分析；运用兼顾急性髓细胞白血病（AML）及急性淋巴细胞白血病(ALL)的治疗方案治疗本组4例BAL患者。目前，我们发现BAL患者约占同期诊断的急性白血病的9.6%。BAL患者的临床表现与AML及ALL患者的差异无显著意义；形态学上BAL表现为AML-M2、M5和ALL-L3；免疫分型显示BAL患者中以髓系、B系共表达者多见。另外，HLA-DR、CD34在BAL中呈高表达，提示BAL的白血病细胞可能起源于早期造血细胞；核型分析可见异常染色体的出现，但本课题的研究尚无法确认其染色体核型异常是否具有特异性。BAL患者对治疗反应差，病情凶险，预后差。接受治疗的4例BAL患者中，首次化疗1例达完全缓解，占25%。另两例在第二次化疗后达完全缓解，剩下一例进行了第3、4疗程仍未缓解。
